Independent Contractor Information
IMPORTANT: Please carefully review the information below regarding your submission before submitting your information.
1. Independent Contractor Relationship
You understand and agree that, if engaged, you will perform services as an independent contractor and not as an employee of Lux Medical Logistics. Nothing in this submission, or any subsequent agreement, is intended to create an employer–employee relationship, partnership, joint venture, or agency relationship.
2. No Employment Benefits
As an independent contractor, you will not be eligible for employee benefits of any kind, including but not limited to:

Retirement plans

Paid time off

Workers’ compensation coverage

Unemployment insurance
3. Control and Discretion
You will retain control over the manner and means by which you perform services, including (where applicable):

Selecting your own work schedule

Determining how services are performed

Supplying your own tools, equipment, and resources
The Company may specify desired outcomes or service requirements, but will not control the day-to-day performance of your work beyond what is necessary to ensure compliance with applicable laws, contracts, or client obligations.
4. Business Independence
You represent that you are operating as an independently established business or trade, and you may:

Provide services to other companies or clients

Advertise and offer your services independently

Accept or decline work opportunities at your discretion
5. Tax Responsibility
You acknowledge that you are solely responsible for:

Reporting and paying all applicable federal, state, and local taxes

Self-employment taxes, if applicable

Any required licenses, permits, or registrations
The Company will not withhold taxes or make employer tax contributions on your behalf.
6. No Guarantee of Work
Submission of information does not guarantee engagement, assignment, or any minimum amount of work. Any services performed will be governed by a separate written agreement, if applicable.
